Mandatory Clinic Selection:

To help us coordinate your care, as of April 1st, 2026, all new and existing appointments must be scheduled at a SINGLE, chosen clinic location (Kogarah, Bondi Junction, or Cronulla). Patients can no longer alternate locations.

To confirm your appointment, you will be required to complete the online forms listed below and submit them to the Clinic  at least 5 working days before your scheduled appointment.

(1) New Patient Form. This allows us to set you up in our system for e-scripts if needed as well as providing us with important information prior to your appointment.

(2) Food & Cravings Questionnaire. Screens for the intensity and frequency of cravings, and cues that trigger the cravings

(3) Food Noise Questionnaire. Screens for intrusive +/- persistent thoughts about food and eating, which interfere with daily life and healthy behaviours.

(4) DASS-21 Questionnaire. Screens for depression, anxiety, and stress.

(5) Privacy Policy. We value your privacy and would ask that you carefully read our Privacy Policy.

These forms will be reviewed by Dr. Rigas before your consultation. They help to tailor treatment to your particular health profile; in addition to being helpful tools to measure changes (if any) as a result of treatment.

IMPORTANT: If we do not receive all responses prior to the deadline, you will be asked to reschedule your appointment.

While a GP referral isn’t required, it can be helpful, as would a copy of your most recent blood test results (even if they’re over a year old).  Please forward any other medical reports that you feel would be useful for Dr Rigas to view during your appointment with her.

If you need to cancel an appointment

If you are running late for your appointment, kindly ring the Clinic and let us know. However, if you are late for your appointment, it means that there will be less time in the consultation with Dr Rigas.

We recognise that your time is valuable, and we make every effort to run to time. Occassionally, emergencies arise OR patients require a little bit more time. These can cause scheduling delays beyond our control. We apologise if we keep you waiting.
